چکیده

Since the first human electroencephalogram (EEG) was recorded in 1929 by Hans Berger, enormous advances have been made in EEG recording technology and data analysis [1, 2]. The recording period was extended and long-term EEG monitoring became technically feasible when computer applications were introduced in the 1970s and digital EEG recording systems established. However, the use of long-term EEG monitoring was mostly limited to epilepsy monitoring units and only subsequently found its way to the intensive care unit (ICU). Yet, an old comparison to cardiology still holds true: While it is an unchallenged standard to continuously monitor heart function in all ICU patients with abnormal cardiac function, brain activity is often not monitored in patients with acute brain damage, mainly because brain function has been more difficult to monitor. A single or even repeated EEG represents only a small sample of data. For example, seizures in the ICU setting are usually brief and paroxysmal and can easily be missed when EEG recording is performed intermittently. Fortunately, over the past 15 years, rapid advances in computer technology and digital data transmission and analysis have made it possible to introduce continuous EEG monitoring in the ICU. The goal of continuous EEG monitoring is to enable intensive care physicians to predict impending central nervous system (CNS) injury at a time when intervention is still possible. The challenge for the future will be to demonstrate that intensified monitoring of brain function by continuous EEG will result in better outcomes.

Status epilepticus on the paediatric intensive care unit—the role of EEG monitoring

A pilot study was undertaken of the feasibility of continuous EEG monitoring of patients admitted to a Paediatric Intensive Care Unit (PICU) for management of status epilepticus or its immediate sequelae. Eight children were studied and seizure activity was recorded in four patients. Additional information influencing management was obtained: the bedside nurse considered decerebrate posturing i...

O16: Long Term Video EEG (AC/DC) Monitoring

Long term video electro-encephalographic (EEG) monitoring (LTM) is defined as the continuous and synchronized recording of EEG and multimedia to analyze brain abnormalities. A conventional LTM system continuously records EEG in the frequency range of 0.5-70Hz. The data synchronization, the high-volume data management, the system reliability as well as noise reduction remain significant challeng...

Effect of p-CPA Pretreatment on EEG Power Spectra in Experimental Open Brain Injury in Rats

Continuous four hours EEG (electroencephalogram) recordings and its power spectrum analysis using fast fourier transform (FFT) in urethane anesthetized male Charles Foster rats were performed in two groups: open brain injury and p-CPA (para-Chlorophenylalanine) pretreated before brain injury, respectively, and compared with the EEG power spectrum of control rats. The EEG power spectrum analysis...

Bispectral index versus COMFORT score to determine the level of sedation in paediatric intensive care unit patients: a prospective study

INTRODUCTION Most clinicians give sedatives and analgesics according to their professional experience and the patient's estimated need for sedation. However, this approach is prone to error. Inadequate monitoring of sedation and analgesia may contribute to adverse outcomes and complications. With this in mind, data obtained continuously using nonstimulating methods such as bispectral index (BIS...

An expert system for EEG monitoring in the pediatric intensive care unit.

OBJECTIVES was to design a warning system for the pediatric intensive care unit (PICU). The system should be able to make statements at regular intervals about the level of abnormality of the EEG. The warnings are aimed at alerting an expert that the EEG may be abnormal and needs to be examined. METHODS A total of 188 EEG sections lasting 6 h each were obtained from 74 patients in the PICU. F...
